Service Organization Agreement By submitting this profile, the organization (the "Organization") asks Evangelicals for Social Action / Fresno Inc., also known as ESA / Love INC ("ESA"), to sign up the Organization in Fresno County. 1. The person submitting this form represents that they are authorized to sign up the Organization. 2. After ESA staff approve the application, ESA may list the Organization's name, logo, website, and city on esali.org as a community partner. 3. ESA may add the primary contact's email address and phone number to the ESA newsletter list and send a confirmation email when the signup is approved. 4. Signup does not transfer control of the Organization, does not make the Organization an agent of ESA, and does not require the Organization to provide any service it did not list. 5. The Organization will keep its public contact information accurate and may ask ESA to update or remove the listing by emailing loveinc@esali.org. 6. ESA may decline an application or end a partnership. 7. The Organization also accepts the Terms of Service and the Privacy Policy shown with this form.

These terms and conditions (“Agreement”) set forth the general terms and conditions of your use of the esali.org or lovenet559.org website (“Website”), “ESA Love Inc. Community Program” mobile application (“Mobile Application”) and any of their related products and services (collectively, “Services”). If you are entering into this Agreement on behalf of a business or other legal entity, you represent that you have the authority to bind such entity to this Agreement. If you do not agree with the terms of this Agreement, you must not accept this Agreement and may not access and use the Services. By accessing and using the Services, you acknowledge that you have read, understood, and agree to be bound by the terms. We connect people with needs to social service providers within Fresno County through the local church. We are also known as ESA/Love INC. This Policy describes the information we collect on esali.org and lovenet559.org and how we use, protect, and share it. For an organization signup, we collect the organization name, primary contact, email, phone, address, services, who you serve, how services are provided, website, and logo. After approval we use the email and phone for the newsletter and publish the name, logo, and website on the community partner page. We do not sell personal information. You may ask us to update or remove the listing by emailing loveinc@esali.org.
